Under 18s
In the event of a medical or child protection issue, the Club may disclose certain information to medical specialists and/or to police, children’s social care, the Courts and/or probation officers and, potentially to legal and other advisers involved in an investigation. As the person completing an application, you must ensure that the person whose information you include in this application knows what will happen to their information and how it may be disclosed. By completing an on-line application form, you agree to the child in your care taking part in the activities of Blueharts Hockey Club. And you understand that in the event of injury or illness, all reasonable steps will be taken to contact you and to deal with that injury/illness appropriately.
Disclosure
Any information you provide to us will only be used by us and possibly passed to our partners for the purpose of dealing with payments and using communications portals. It may also be disclosed to your organisation’s Governing Body. You should also be aware that your information may be disclosed where we are obliged or permitted by law to do so.
Timescale
We will hold your Personal Information within our systems for as long as you remain a member of the club or want to be kept informed of club activities. If you cease to be a member or no longer wish to be kept informed of club activities all personal data will be deleted within 6 weeks of the date you inform us.
